Health & Safety Incident Reporting
Effective September 3, 2019, the Rainy River District School Board has implemented the eBase Online Incident Reporting Program. This program is to be used to submit Employee Accident/Incident/Near-Miss reports and Safe Schools Incident Reporting Forms.

Hard copies of the Employee Accident/Incident/Near-Miss Reports and Safe Schools Incident Reporting Forms will remain available for completion in the event the online program is not available (ie. internet access, invalid login credentials, etc.). Hard copies of the incident reporting forms are available at your work location's Main Office, in the FirstClass "Forms" folder, and in the "Forms" section of the Board website.
